• A scan/photograph of your last official school transcript to upload into the application form – this can be done by taking a photo of the document with your phone or by using a scanner.
• Your secondary school report card(s) can be obtained from the school board you attended.
• Transcripts issued by the Ministry of Education of Quebec can be requested through our Administrative Services.

In Chronological Order. For each experience you will need to include:

• Name & address of organization and name of supervisor or employer & phone number
• Position Held
• 3 main responsibilities relevant to social work
• From when to when (year, months, etc.)
• Paid or volunteer
• Total hours

• Describe why you are interested in becoming a social worker and how aspects of your own identity and/or past experiences (personal, academic, volunteer and/or work) have contributed to your interest in social work.
• Reflect on what you have learned from these past experiences that make you a good candidate for social work.
• If you have worked in other fields/disciplines, please share the reasons for this change.
